We are committed to scaling impact in maternal, newborn, and child health. Under the Hospital-to-Home Model of the Uganda Newborn Programme, led by ADARA Development Uganda, we are strengthening the continuum of care from health facilities to households, ensuring that every mother and newborn receives the support they need during the critical early days of life.
This model focuses on
• Continuity of Care: Linking hospital-based interventions with follow-up at the community and household level, ensuring that newborns discharged from neonatal units continue to receive care and monitoring at home.
• Empowering Families: Training caregivers in essential newborn care, Kangaroo Mother Care, and recognition of danger signs to improve survival and health outcomes.
• Strengthening Health Systems: Working with health workers and community volunteers to provide mentorship, data tracking, and timely referrals, bridging gaps between hospitals and communities.
• Evidence-Based Innovation: Using data and digital tools to monitor outcomes, track progress, and inform policy, enabling the programme to expand to high-burden districts efficiently.
Through this approach, we aim to reach more families, prevent complications, and ensure that newborns thrive beyond the hospital walls, turning interventions into lasting impact in communities across Uganda.
The model was first piloted at Kiwoko Hospital, where community health workers received intensive training, supportive supervision, and monthly peer engagement.
These frontline workers became trusted extensions of the hospital delivering lifesaving care at the household level.
This model is also being piloted in Nakaseke hospital, a government hospital and the findings will guide scale up to government facilities by Ministry of health and partners.
